Program Participation Enrollment and Evaluation Process
All persons over the age of four and diagnosed with any physical, mental, emotional, or cognitive disability are eligible to participate in our programs. Once all necessary paperwork is received prospective clients are evaluated on a case-by-case basis to ensure their safety and to determine the best programming for that individual.
To get started, please complete all of the online forms below. If you are applying for our Thornbrook Farm location please note that an additional release must be submitted to comply with the farm's policy.
Please Note: Access Equestrian and PATH, Intl. require a Physician Release to ensure their safety in activities involving equine assisted participation, which may be accessed by clicking, here. This form must be printed and filled out by the prospective participant's doctor in order for a participant to be able to ride. However, all other required forms may be accessed below and submitted online.
Please Note: Prospective participants applying for services at our Thornbrook Farm location must also complete the Thornbrook Farm Equine Activities Waiver & Release listed below.
